POSITION SUMMARY
This position is responsible for providing community outreach to the unsheltered population to assist with housing and connection to services. The PATH program serves individuals with mental health disorders who are homeless or at risk of homelessness. Duties include connection to care, assistance accessing benefits, and general case management support. This is a mobile position with non-traditional hours and requires attendance and participation at local provider meetings.
Hourly rate based on experience $16.00 - $17.66
Effective immediately, all new hires must be fully vaccinated against COVID-19 prior to start date. Limited exemption may be considered for veified medical and religious reasons only.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Perform outreach activities to individuals who are currently, or at risk of, experiencing homelessness.
- Provide linkage to mental health and substance use treatment services, including a wide range of treatment modalities and recovery communities.
- Identify needs and assist individual in gaining access to financial benefits and other supportive services including but not limited to medical, social, housing, family planning and reunification, and educational services.
- Provide education and coaching, particularly in available resources within the local recovery community.
- Monitor and track client progress.
- Assist with obtaining reliable means of transportation, including use of public transportation. Provide transportation when appropriate and required by program.
- Upload data into state and federal databases on a regular basis.
- Attend and participate in local provider meetings.
- Perform other duties as assigned.
MINIMUM QUALIFICATIONS
- Associates degree in related field. Relevant experience may be considered in lieu of education.
- Minimum of 3 years’ experience in related field.
- Valid driver’s license.
- Prior knowledge of mental health and substance use strongly preferred.
- Bilingual Spanish preferred.
- Lived experience with mental health and/or substance use disorders strongly preferred.
- Ability to learn new systems and programs quickly.
- Excellent verbal and written communication skills.
- Excellent customer service skills.
- Ability to develop clear and professional documentation.
- Willingness and ability to establish and maintain healthy working relationships.
