Job Description:


The Shared Living program serves 60 adults with varying challenges and ages to become part of a host family, with all the benefits and responsibilities that sharing a life and a household entail. Host families, often referred to as providers or caregivers, help the individual to build meaningful relationships and provide opportunities for them to lead their best life.  We value the diversity and culture of each home and design tailored supports for the provider depending on everyone’s unique needs and preferences.  Open Sky’s Shared Living program allows providers to gain the satisfaction of knowing they are helping someone to develop the tools to live a meaningful and satisfying life. In exchange, Open Sky provides each home a comprehensive support team including individualized case management, clinical, nursing supports and a generous monthly tax-free stipend.

The Shared Living Coordinator operates out of Whitinsville and maintains a caseload of placements in the Central Massachusetts region. They provide support and training to Shared Living Providers and assist in the facilitation of meaningful community roles and relationships for individuals served.

Other Key Responsibilities:

  • Support and monitor Shared Living placements through regular home visits, provider coaching, and ongoing quality oversight.

  • Develop and implement Individual Support Plans that promote personal growth, independence, and community inclusion.

  • Coordinate clinical, medical, and behavioral supports to ensure individuals receive appropriate care and services.

  • Collaborate with families, guardians, service providers, and funding agencies to support individuals' goals and needs.

  • Maintain accurate documentation, regulatory compliance, and oversight of individuals' funds and related records.

  • Participate in on-call rotation and provide crisis response and support as needed.

Qualifications:

  • Bachelors degree in Human Services or related field and/or two years residential or administrative experience, required.

  • Valid Driver’s License, acceptable driving record, and reliable vehicle with proof of insurance required.  


Pay Range:

$23.00 - $25.87
