Description
Job Duties:1. Participates as a team member to provide a nurturing and consistent program environment.
2. Ensures assistance with personal hygiene is provided as needed in a safe and respectful manner
3. Establishes and follows appropriate admission, discharge, and referral procedures for all IDD programs and services.
4. Ensures services as approved on the Service Authorization are provided and incorporated in the Plan for Services and Supports and the IDD Activity Support Plan.
5. Provides and coordinates necessary training for program staff. Participates/conducts evaluation of program staff to determine competency and the provision of quality care.
6. Observes individuals to determine need for adaptive equipment or program modifications. Communicates needs/modification to the Director of IDD services, as well as, to be included in the Plan for Services and Supports as appropriate.
7. Monitors and orders program supplies, office supplies, food, furniture and equipment in a cost effective manner while ensuring that individual’s needs are met.
8. Monitors program equipment and/or supervises staff or individuals operating program equipment in a safe and competent manner.
9. Monitors facilities and programs to ensure a clean and safe work environment.
10. Addresses and corrects equipment failure and safety concerns in the workplace.
11. Reports any unusual situations/incidents, safety concerns, or changes in an individual’s behavior to the Director of IDD Services as necessary. Makes additional reports to Support Coordinators/Targeted Case Managers as indicated/required.
12. Ensure drills are conducted in compliance with DMH standards for all IDD programs/services and appropriate documentation is maintained.
13. Acts as a liaison with families and referral sources to promote the well-being of the clients.
14. Maintains client confidentiality at all times.
Documentation and Quality Improvement:
15. Ensures a case file is maintained on each individual in compliance with DMH standards; monitors IDD Activity Support Plans and progress notes for adequate completion.
16. Monitors case records on a regular basis and ensures that all deficiencies are corrected in a timely manner.
17. Assists in the development and implementation of Plan of Services and Supports and the IDD Activity Support Plan.
18. Documents relevant events in a factual, accurate, complete, and timely manner.
19. Monitors program attendance/participation and ensures accuracy of documentation submitted for billing.
Interpersonal and Leadership Skills:
20. Provides clinical consultation for treatment team members. Coordinates treatment team meetings and ensures activities as outlined on the IDD Activity Support Plan are being implemented.
21. Participates annually in the budget process for all IDD Services/Programs and maintains budget compliance throughout the fiscal year.
22. Responds to individuals needs in an appropriate and timely manner.
23. Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.
24. Seeks professional certification and/or licensure and clinical training opportunities.
25. Other duties as assigned by supervisor.
Supervised/Supported Living
26. Provides a safe, secure, and clean 24 hour group home environment for a designated population.
27. Implements an organized, structured, therapeutic program based on individual’s needs over a 24 hour/day, 7 day/week period under the direction of the Director of IDD Services. Plan activities that support meaningful days for each person. Design activities that promote independence and personal choices while providing necessary support and assistance.
28. Implements a 24 hour staffing schedule that ensures adequate supervision of individuals, program compliance, and compliance with DMH standards and approved by the Director of IDD Services.
29. Monitors facility periodically and routinely on evenings, nights, and weekends to ensure safety and program compliance.
30. Maintains an average monthly census in the program of not less than 90%.
31. Implements a program that ensures that the medical, therapeutic, social, educational, and spiritual needs of the individuals are being met.
32. Ensures that each person has adequate clothing and personal hygiene supplies.
33. Ensures that program vehicles are clean and in good repair. Reports concerns in a timely manner.
34. Oversees and reconciles a petty cash account in compliance with agency accounting policies.
35. Provides for the safe and appropriate storage and monitoring of each individuals medication.
36. Displays all certifications and licenses appropriately as required by standards.
37. Designates an appropriate staff member in charge at all times, as well as, an on-call rotation for emergencies.
Day Services-Adult/Day Habilitation Services:
38. Develops and monitors a structured day program in conjunction with the Director of IDD Services and in compliance with DMH standards. Supervises program to ensure compliance.
39. Monitors referrals of individuals to state regional centers for determination of program eligibility.
Pre-Vocational/Work Activity Services:
40. Implements a program that provides meaningful day activities of learning and work experiences where individuals can develop skills that contribute to employment in an integrated, competitive work setting.
41. Ensures employment related outcomes are outlined on the Plan for Services and Supports and that activities are designed to support employment outcomes.
42. Incorporates programming that includes monthly job exploration activities.
43. Monitor progress towards employment related outcomes.
44. Maintains a Work Activity program so skills developed in prevocational services can be practiced.
45. Provides program orientation to clients in a timely manner.
46. Ensures time studies are conducted as needed and that accurate records of all contract work activity are maintained.
47. Implements a plan for completing contract work in compliance with contract specifications and time line specifications. Oversees the assignment of work activities based on individual abilities.
48. Maintains documentation in compliance with Department of Labor, OSHA, and WIOA standards.
49. Monitors records of individuals work activity and ensures information is submitted to payroll in a timely manner for processing.

Requires completion of annual training in compliance with DMH standards.
Requires working knowledge of the DSM.
