Job Description

Under the direction of the Clinical Pharmacist, this position is responsible for supporting the pharmacist in managing medication adherence initiatives and pharmacy quality measures, including CMS Star Ratings. This position is responsible for identifying patients at risk for non-adherence, conducting patient outreach, coordinating medication access solutions, documenting interventions, and assisting with quality improvement activities designed to improve patient outcomes and organizational performance.  The Pharmacy Technician serves as an integral member of the population health and quality team, working collaboratively with pharmacists, providers, care managers, health plans, and patients to close medication adherence gaps and improve performance on Medicare Star measures.   The Pharmacy Technician will interact with other departments, clinic personnel, and outside providers in a professional and friendly manner, to create and maintain a positive relationship with our internal and external customers.

 

 

 

Care Coordination:

1.1 Collaborate with pharmacists, providers, medical assistants, care coordinators, and health plan representatives.

1.2 Assist with obtaining prescription renewals and refill authorizations.

1.3 Coordinate medication access programs, copay assistance, and patient assistance resources.

1.4 Identify patients requiring pharmacist intervention and facilitate warm handoffs.

Data management & Reporting:

1.5 Maintain accurate records of outreach activities and intervention outcomes.

1.6 Assist in developing adherence tracking reports and performance metrics.

1.7 Support audits and quality assurance reviews.

1.8 Monitor work queues and ensure timely follow-up on identified gaps.

 

Medication Adherence Management:

2.1 Review adherence reports and pharmacy claims data to identify patients at risk for non-adherence.

2.2 Conduct outbound patient outreach via telephone, text messaging, mail, and other approved communication channels.

2.3 Assess barriers to medication adherence including cost, transportation, side effects, forgetfulness, and access issues.

2.4 Escalate clinical concerns and medication-related questions to the supervising pharmacist.

2.5 Assist patients with medication refill coordination and prescription synchronization.

2.6 Facilitate conversion to 90-day medication supplies when appropriate.

2.7 Coordinate mail-order pharmacy enrollment and medication delivery services.

2.8 Facilitate provider and pharmacy outreach with ability to communicate or triage medication requests.

2.9 Document all patient interactions and interventions in the designated systems.

 

CMS Star Measure Support:

               

                2.10 Support adherence initiatives for:

  • Statins
  • Diabetes medications
  • Renin-Angiotensin System (RAS) antagonists

                2.11 Monitor adherence performance dashboards and patient gap reports.

                2.12 Participate in quality improvement projects designed to improve Star Ratings performance.

                2.13 Participate in weekly or monthly meetings with health plans to address adherence gaps and areas for improvement.
