Job description

The Program Coordinator works in collaboration with the Residential Director and Team Leaders to ensure the effective and efficient management of daily programmatic operations for the Delaware residential homes. In particular, the Program Coordinator coordinates person centered services to individuals supported, as well as all other formal and informal support available to the individual in their home, Day Program, and their community. The Program Coordinator partners with the Team Leader to co-manage an assigned home, and to provide supports to the  Direct Support Professionals, to assure programmatic integrity in adherence to the Division of Developmental Disabilities Services (DDDS) provider standards.



The incumbent is accountable to the Residential Director and the Delaware Residential Programs and:




  • Maintains on-going Outlook Calendar to include home QA visits to homes to include weekend hours and hours outside of 9 AM to 5 PM on weekdays.  Completes Monthly DE QA Audit as assigned by the Director.  Visits assigned individuals residing in alternate living arrangements to include hospitals, and rehabilitation as applicable.  Maintains quarterly visits to individual’s supported day program;

  • Interfaces with DDDS State offices and various agencies (Job Coaches) as needed to make sure persons served receives the needed and desired services/supports.  Coordinates and participates in team meetings conducted by the DDDS Supports Coordinator for the individuals as required;

  • Point Person with families and other interested parties.  Attends Person Centered Planning meetings and Team Meetings to monitor and maintain the responsibility of responding to related to the above monitoring in their Caseload;

  • Ensures that families, advocates, psychiatrists/physicians, psychologists, nurses, speech/physical/occupational therapists, residential and day program staff are involved in the development and review of the PCP and are notified of the Annual meeting;

  • Completes all necessary preparations for meetings and attends all PCPs as well as the internal/external interdisciplinary team meetings. 

  • Maintains compliance with specific licensure requirements as stipulated in the DDDS Provider standards.   

  • Attends, maintains and responsible for all associated follow up for pre planning meetings. 

  • Implements the PCP and other treatment plans.  Reviews Individual outcomes and constantly updates the PCP Programs in THERAP for all individuals in the caseload.

  • Provides effective training, mentoring supervision and monitoring DSP as pertaining to documentation of individual outcomes as per their respective PCP.

  • Review Personal Spending Records (PSR) for the individuals monthly to ensure the agency is meeting the DDDS provider standard. This includes reviewing all documentation in Therap as it relates to uploading receipts, cash on hand, banking deposits and withdrawal of the Individuals community bank accounts. Complete Quarterly PSR reviews in THERAP for each Individual.

  • Conducts Annual Site-Specific Trainings for all DSPs in the Caseload to ensure compliance with DDDS Provider Standards, and thereafter as needed especially when we have a new employee or a staff assisting to cover open shifts.  Provides support to Direct Support Professionals, especially on quality-of- service notes/documentation, to enable timely billing in THERAP.




  • Assists the Director in all efforts made to fill program vacancies or to expand services.  May perform other duties as assigned.

Qualifications

Minimum Qualifications: 




  • Bachelor’s Degree and 3 – 5 years’ work experience with individuals with disabilities; Work experience preferred in one of the following areas:  Human Services, Special Education, Social Work, Psychology or Social Rehabilitative Services with people with disabilities preferred or as required by the State of Delaware;

  • Knowledge of division rules, regulations, and policies concerning care, treatment, and rehabilitation in community settings.  Knowledge of principles, methods, devices, and techniques involved in case management, analysis, development, and implementation of programs for people with intellectual disabilities and mental health challenges. 

  • Working knowledge and experience with on-site residential programs;

  • Valid local driver’s license with good driving record along with valid and reliable transportation;

  • Strong communication skills, oral, written and prepares/delivers presentations.  Ability to read, write, speak, understand, or communicate in English sufficiently to perform the duties of this position. 

  • Ability to organize assigned caseload and develop appropriate work methods.  Works with various levels of professional staff concerned with the care, treatment, and support of individuals;

  • Develops and trains staff in various strategies for the support of the individuals;



 



Implementation skills and communicates plans for services while meeting all regulatory requirements; 



Ability to design, implement and explain plans to the staff, parent/guardian designed in the best interests of the assigned individuals;



Ability to draft and analyze reports dealing with the assigned caseload; ability to prepare correspondence and reports containing findings, conclusions, and recommendations;



Ability to maintain records and files and the ability to learn and utilize various types of electronic and/or manual recording and information systems used by the agency, office, or related units;



Knowledge of services available to individuals in a day or residential program(s).



 



 



This position may be eligible to participate in a hybrid ((partial tele-work) model upon approval by the Managing Director. A minimum in-office presence is required. Incumbent may be required to perform scheduled on-call duties during off-hours. All employees of Community Interactions, Inc. are required to successfully complete pre-employment physicals, including drug testing, as well as Nationwide and local criminal background checks as dictated by licensing bodies, regulations and/or our insurance carrier.
