$22-$28/hr

Responsibilities

  • Intensive Case management (for approximately 20 individuals and their families) navigating alongside customers to alleviate the barriers to long term unemployment/underemployment and increase family stabilization using Family Development strengths based approach. Triage case load to ensure adequate support to families in crisis and high needs families.
  • Complete comprehensive assessments of family strengths and needs using agency tools and best practices specifically related to substance abuse, mental health, developmental/learning challenges and career assessments. Develop goals and milestones with family participation according to the Brief Solution Focused treatment model and Family Development in conjunction with the Employment Services Team. 
  • Conduct regular and up to date evaluation of progress and modify goals on a periodic basis to provide support for successful completion of training and employment, do this independently and effectively communicate and work with a team – both in- house and through case conferencing as appropriate. 
  • Ensure comprehensive access and follow-through to all of SCAP’s services, joining the customer in working with agency colleagues and community partners to fully integrate, streamline and coordinate family services. 
  • Conduct advocacy on behalf of families in a solution-focused manner and represent SCAP in a positive, professional manner. Develop and maintain good, effective working relationships with community providers to include jails, prisons and community mental health providers. 

Qualifications

Requirements

  • Bachelor’s Degree in social work, human services or related field, Master’s Degree preferred.
  • Two (2) years’ experience providing related services to families and individuals, one (1) year experience with master’s degree. Experience in evidence-based practices preferred.
  • Knowledge of community-based services, public assistance and other community resources and demonstrated skills in accessing these services appropriately.  Must have the ability to learn quickly the resources available in Schenectady County and be able to appropriately refer in-house working with other program support staff to ensure full access to SCAP’s services.
  • Must have demonstrated written and verbal communication skills.
  • Must have demonstrated ability to perform job responsibilities independently.
  • Must have a valid New York State driver’s license, have regular access to a car and be willing to travel to off-site locations.
  • Demonstrated computer expertise with Microsoft Office and be able to stay up to date on software used in best practices.
  • Background check is required through the NYS Central Registry of Child Abuse. 
  • Knowledge and understanding of issues related to individuals and families in poverty.  Ability to build upon the strengths of those we serve with diverse racial, cultural, social, educational and economic backgrounds, including high risk populations, to provide high-level services and lasting solutions to fit each individual’s and family’s needs.
  • Must be willing to work a flexible schedule to accommodate family needs.
