What You Will Do:

  • Initiate, support, and ensure quality of life through empowerment, safety awareness, and respect and dignity.
  • Ensure overall health and well-being of individuals served by actively participating in Multidisciplinary Team meetings, accompanying individuals to medical appointments, and arranging needed follow-up.
  • Oversee all aspects of medication treatment and administration, including preparing Health Care Provider Orders, Medication Administration Records, Current Medication Lists, Weekly and Monthly MAP Checklists, and Medication Occurrence Reports (MORs), and ensure corrective action required by a MAP Audit occurs.
  • Provide crisis intervention and emergency services as needed, including serving as a member of the on-call team.
  • Act as a primary implementer of strategies to meet Individual Service Plan (ISP) goals.
  • Take necessary actions to protect individuals served in the event of an abuse/neglect complaint and complete all required follow-up from internal and external investigations.
  • Complete all documentation required within the electronic health record.
  • Review portions of the electronic health record prepared by supervisees in a thorough and timely manner.
  • Complete program-specific forms and required time-sensitive documents in a timely manner and ensure proper review before submission, including Health Encounter Forms and Contract Reports.
  • Complete health and medication-related documents as individually indicated and in accordance with BHN policy and MAP regulations.
  • Update and maintain the electronic health record as needed.
  • Prepare documentation for the ISP process, including annual assessments, goals/objectives, support strategies, and progress summaries as required by BHN.
  • Ensure communication occurs between staff, the program, outside agencies (including health care providers and pharmacies), and families.
  • Recognize and report program-specific safety and maintenance concerns and ensure property is safe and well maintained.
  • Ensure that fire drills are conducted as required by the Safety Plan.
  • Ensure that individuals served receive benefits management, including support to maintain Social Security, health insurance, food stamps, and housing subsidies.
  • Follow agency policies with licensing and certification requirements.
  • Regular and punctual attendance is essential for this role.
  • MAP certification required within the first 6 months of employment.
  • Provide direct services that ensure health and safety, support growth, and promote independence of individuals served.
  • Provide indirect services that document and support the provision of direct services to individuals served.
  • Complete individually assigned program-specific responsibilities.
  • Provide supervision and complete administrative duties to ensure individual health, safety, and well-being.
  • Attend in-services, meetings, and trainings as assigned and scheduled; keep mandatory trainings up-to-date and monitor employee training completion.
  • Other duties as assigned.

Who You Are:

  • High School Diploma or GED plus 3 years relevant experience required, OR Associate's Degree or equivalent required; Bachelor's Degree preferred.
  • Minimum of two years' experience in human services.
  • Supervisory experience preferred.
  • Valid driver's license and reliable car available for all working hours required.
  • Committed to individual empowerment and growth, human rights, and community integration.
  • Good organizational skills.
  • Excellent interpersonal, verbal, and written communication skills.
  • Familiarity with internet, word processing, electronic file management, and email systems.
