Job Description:

The Stroke Program Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The Stroke Program Coordinator is an experienced clinician to oversee the care of our stroke patients. This key team member performs duties in support of and in compliance with The Joint Commission, other licensing/accreditation bodies, and performance improvement plans. The position requires data collection, analysis, continuous quality improvement, patient satisfaction initiatives, and community outreach, along with patient guidance, education, and follow-up for our stroke patients. The position works collaboratively with staff and physicians to create seamless coordination of care for the stroke patient population.

Qualifications:

Required:

  • Minimum of 5 years RN experience
  • BSN
  • Current State of Illinois RN License
  • NIHSS certification
  • Previous stroke experience
  • Proficient in Microsoft Word, Excel, PowerPoint

Preferred

  • Familiarity with TJC DSC for stroke
  • CNS/APN
