Oklahoma Human Services: Administers social services and public assistance programs in Oklahoma.
1+ YOEBachelor's in a human services field plus 1 (Level I) or 2 (Level II) years' professional experience with individuals with intellectual/developmental disabilities or related fields, or Oklahoma RN license with equivalent experience; complete Case Manager Academy; valid driver's license and insurance; extensive travel.
Good Shepherd Hospice: Provides compassionate, home-centered hospice and palliative care services.
Active RN license in care state, hospice/home health or geriatrics experience preferred, commitment to hospice philosophy, performing assessments, care planning, patient/family education, and documentation.
A Path of Care: Provides skilled nursing and therapeutic care in patients' homes.
Current RN license required; strong emotional intelligence, communication, time‑management, problem‑solving and critical‑thinking skills; ability to work independently and coordinate patient‑centered home care.

Provide case management and career navigation to job seekers, develop service plans, conduct outreach and home/site visits, document case notes, and coordinate service providers.

Active RN license, CPR, valid driver’s license and auto insurance; hospice clinical expertise, strong communication, ability to make independent decisions; HomeCare HomeBase experience preferred.
