Bachelor's or master's in a human services field required; valid driver's license required; emergency/crisis intervention skills, computer and electronic records proficiency, strong verbal/written communication, and ability to document and coordinate services.
Transcend Hospice: Provides compassionate hospice and palliative care services at home.
Current state RN license and valid driver's license; graduate of an accredited nursing program; patient assessment, care coordination, documentation, and end-of-life/palliative skills; reliable transportation and auto insurance; ability to lift up to 50 lbs.

Performs comprehensive public health social work and case management, including home visits, client assessment, service coordination, documentation, community outreach; requires current unrestricted Texas social work license and CSWE-accredited bachelor's degree (master's preferred).
